The scorching summers of recent years have brought more than just discomfort; they’ve taken a silent toll on public health. A groundbreaking investigation by NPR and Boston University has uncovered that heat-related deaths in the United States are vastly underreported, with thousands of lives lost each year not being counted in official statistics.
As climate change intensifies, so does the danger posed by extreme heat. The traditional methods of tracking heat-related fatalities fail to capture the full extent of this growing crisis. This investigation aims to shed light on the true scale of the problem and the reasons behind the undercount.
The Scale of the Undercount
The Centers for Disease Control and Prevention (CDC) reports an average of around 700 heat-related deaths per year from 2004 to 2018, with recent years seeing an increase to about 1,700. However, the NPR and Boston University analysis paints a starkly different picture. On average, between 2018 and 2025, the investigation estimates that approximately 9,000 deaths each year are influenced by heat—five times higher than the official average.
The true figure could be even higher, according to Quinn Adams, a research scientist at Boston University’s Center for Climate and Health. The investigation’s statistical model assessed nearly every county in the country, providing a more comprehensive national count. This analysis will soon be published in a scientific journal, offering a critical assessment of the true scope of heat-related fatalities.
The Silent Killer
Heat is often referred to as the silent killer unlike more visible natural disasters like hurricanes and typhoons. Its role in a death is frequently masked by other more obvious causes, such as heart attacks. Dr. Sameed Khatana of the University of Pennsylvania‘s Perelman School of Medicine explains that heat can trigger a fatal event in individuals with preexisting health conditions.
“The heart starts beating faster and harder to shunt blood to the skin to cool the body,” Khatana says. “Eventually, if someone’s heart is already weak, it may not be able to keep up with the demand that was required of it.” This can lead to a heart attack, but because heat leaves no obvious mark, it is often not noted on the death certificate. This omission creates a blind spot that obscures the scale of heat’s danger.
National Risks and Unexpected Hotspots
The investigation found that heat is killing people in nearly every corner of the country, not just in traditionally hot states like Florida and Texas. States like Michigan and Massachusetts also see a significant number of heat-related deaths, far exceeding the official reports. For instance, Michigan’s estimated heat-related deaths are 10 times higher than the official numbers, and Massachusetts’s estimated deaths are more than 12 times higher.
Heat can be particularly dangerous for people who are not accustomed to it. In many parts of the country that were not historically hot, infrastructure to keep cool is less common. This lack of preparedness exacerbates the risk, especially as climate change drives temperatures higher. The frequency and duration of heat events have been expanding rapidly, posing a growing threat to public health.
The investigation also highlights the personal stories behind these statistics. Debbie Willson, for example, lost her son Adam to what she believes was heat-related complications. Adam had a genetic disorder that made him highly sensitive to heat. Despite their best efforts to stay cool, the breakdown of their swamp cooler during a heatwave in June 2024 proved fatal. Adam’s death certificate listed “Unknown natural causes,” a common occurrence that underscores the undercounting of heat-related fatalities.
As the dangers from heat continue to grow, accurate reporting and understanding of its impact are crucial for developing lifesaving solutions. The NPR and Boston University investigation provides a vital step in recognizing and addressing this silent but deadly threat.
